And why the job role is available and what skills I would need to have to apply for this job. The role of a database administrator is to: Ensure patient medical records are correctly updated Make appointments Check in arriving patients using an internal database system Problem solving skillsTelephone work both internally and externally Administration Support In this job there are a large number of files that need to be updated because they have incorrect information included in them. This problem is solved by the database administrator who investigates the files and resolves any issues that crop up so that the information in these files is completely correct and filed correctly. Part of the reason this job is available is because clients have a back log to get through.Database Management Sy stem and DataI think this job would be good for me because it is a temporary ongoing position that requires the person that gets this job to be flexible in the tasks asked of them. The Cuban Missile Crisis Essay The Cuban Missile Crisis on October 14, 1962, marked one of the highpoints of the Kennedy administration.   The scandal caused widespread panic and indignation to the American people due to the surrounding environmental factors brought about by the cold war.   It revolved around the building of Russian bases on Cuban soil and the staging of nuclear weapons from these said structures.   One such depiction of this event is the movie Thirteen Days which is a most ample goldmine of organizational communications beautifully recreated on the silver screen. The crisis is viewed from the eyes of one Kenny O’Donnell, a Special Assistant to the President, who begs, barters and steals the information he needs to keep his friend and superior abreast of the volatile situation while not selling out his own ethics in the process.   This film depicts an excellent display of organizational communications in one of the most important and influential social groups during a time of trial and tribulation which brings out the best and worst in people. Organizational communications stems from the fact that large companies need to coordinate efficiently in order to achieve the goals set out.   For example, if a firm with 10,000 employees had to go to the president for each of their issues with work, he or she would never be able to do the tasks set out on the institution of the company.   The president’s plate would always be full and it would keep piling up with the new problems which spring up during operations.   The key to solving this problem is delegation.   The leader gets people he can trust to get the job done and specifies which fields to focus on.   This paper will show examples as based on the movie Thirteen Days and will provide an example in today’s modern setting. Communications can be defined as the meaningful interaction of people exchanging knowledge.   Good communications in business is the flow of information from the lowest employee to the person with the highest position in order to achieve objectives and vice versa.   The head can address the company as a whole but not all the information from each worker goes straight to the top otherwise there would be an information overload. Information gathering techniques can be in the form of feedback or focused group discussions.   The movie showed this as a U2 spy plane flying over Cuba taking reconnaissance photos as part of America’s regular monitoring of the region. In the film, when Russian rockets are discovered in Cuba, the information is sent up the pipeline because it was deemed important enough.   If the intelligence was about how the Russian prime minister drank his tea or whether he lights a cigar the proper way, it would be filed under miscellaneous information which would be pulled up only when the need arose.   The CIA analysts forwarded the pictures and interpretations to their section chiefs who make the decision whether the buck stops with them or not who then forwards it to the Director of the CIA who makes the call if it should be sent up the chain of command.   This signifies efficient leader-member exchanges. To find a solution, both cognitive and affective conflicts often come into play.   Cognitive conflict focuses attention on assumptions that may underlie a particular issue and which are often ignored.1   It was done by Adlai Stevenson, ambassador to the United Nations, when he voiced an option to the president which all of them as advisors were thinking but would not voice out since it would be political suicide.   This type of conflict allows the decision makers to weigh all their options before coming to a decision.   Affective conflict on the other hand deals with input which is detrimental to the solution process.   It lowers effectiveness with the input of distrust and controversy. The joint chiefs of the military acted as such towards the president since he would not give permission to execute the action they deemed to be the â€Å"only† option.   Even when the generals knew that the commander-in-chief would never agree with them, they tried to box him in by aggravating the situation.   For example, they ordered a low level intelligence gathering mission which would attract an armed response from the Russians.   JFK could not hinder them without a valid reason but he did work around them by getting in contact with the pilot who was flying the mission and asking him to not do anything or report instances which would force America to a nuclear war. The contradiction to this crisis was that the US stationed Jupiter missiles in Turkey near the Soviet border.   If this had not been done, they could have prevented the predicament they were in.   There was also a lot of bargaining going on from the president asking his advisors for other options to the ultimate solution which was the compromise reached by both superpowers to end the escalating conflict. In my opinion this movie is a goldmine of organizational communications reference.   It concretely depicts what to do and what not to do when decisions need to be made during â€Å"crunch time†.   It might not characterize every scenario but with a little stretch of the imagination, we can get and expound new ideas which we can utilize in our daily operations. We can learn a lot from this movie and apply it to any organization especially an engineering services firm.   As an example let’s use the solid waste disposal department and that the dump trucks did not come on time.   The men loading the trucks or the foreman even notices this and decides to kick it to upper management since the whole timetable for the project was delayed.   Now the manager has to decide where to get the trucks.   He or she could either, call another company then penalize the contractors with the cost of hiring another firm plus damages and never work with them again or be lenient with the firm and just deduct the penalty from what was supposed to be paid out for the usage of the vehicles. Advisors are called in to weigh options before a decision can be made.   Now if the trucks were delayed within an allowable amount of time, this information can be sent up the ladder depending on the foreman’s judgment.   If it is still within his scope of responsibility, he can give a warning to the trucking company that tardiness would not be tolerated or send it to one of the manager’s advisors or his corresponding supervisor. The chain of command ideally should work this way to achieve goals but other factors come into play.   In some cases the information does not go to the right person or nothing is being done.   We may not have the perfect structure for organizational communications but we can strive and learn from past errors in order to make decisions which the organization can count every time. Oral Anti-diabetic Drugs Chua Mei Yi Several factors that need consideration when comparing two oral anti-diabetic drugs include degree of glycemic lowering needed to attain target goal range, effect of the medication on weight and lipid profiles, contraindications, side effects, cost, and potential level of adherence to the regimen. The two drugs for comparison are namely metformin and gliclazide. Besides having the same level of glycemic lowering percentage, both metformin and gliclazide are also considered one of the least expensive oral anti-hyperglycemic agents (Reinhold Earl, 2014; Mozaffari, 2013; Holt, Cockram, Flyvbjerg Goldstein, 2011). One of the potential problems with anti-diabetic medication, is that the conventional dosage form is given in 2 to 3 doses daily, and it causes a conflict in compliance rate for the elderly who has poor memories. In this circumstance, both metformin and gliclazide offers the option of single daily dose with either extended-release metformin or gliclazide modified release tablets. Of course the side effects, improvements and degree of glycemic lowering are the same as conventional tablets (Unger, 2013; Barnett, 2012). Metformin is well known for being the first line drug against diabetes type II treatment over the past decades for some reasons. First, Metformin as a biguanide, acts on pancreas independently, inhibiting the liver’s production of glucose, thus controlling blood glucose by improving insulin sensitivity and reducing insulin requirement. If it’s taken when blood glucose is normal or low, it does not stimulate insulin release in the pancreas, thus poses little threat of hypoglycemic (Lehne, 2013). Second, Metformin treatment results in a moderate reduction in circulating triglyceride levels, causing an improvement in lipid profiles that leads to the occurrence of weight loss (Goldstein Muller-Wieland, 2013). Third, having a high frequency of GI side effects due to the inhibition of carbohydrates being digested can be minimized by slow-dose titration and by taking the medication with food. No matter how safe a medication is, contraindications and adverse side effects still need to be observed. As for metformin, renal or hepatic dysfunction patients are contraindicated, and lactic acidosis due to metformin’s accumulation, is considered one of the rare but fatal adverse side effect. The only down side of metformin is, it should be withhold prior to radiological procedures involving contrast dye, as it predisposes patients to acute renal impairment (Reinhold Earl, 2014). Gliclazide, on the other hand, is classified as sulfonylureas. It works by increasing insulin release from pancreatic beta cells, driving blood glucose levels down, causing hypoglycemia if taken when blood glucose is normal or low, thus should only be prescribed to patients likely to have regular food intake. Driving or operating machinery during the initial treatment phase needs to be careful as hypoglycemia might occur (Lehne, 2013). Besides having minimal effect on lipid profile, Gliclazide causes undesirable weight gain due to increased insulin secretion and reduced glucose excretion (Meeking, 2011). Contraindications, particularly renal or liver disease patients, as well as rare but fatal adverse side effects of gliclazide, impairment of liver function, must still be observed (MIMS Australia, 2014). In conclusion, while there is significant debate regarding specific treatment for patients with type II diabetes, most experts agreed upon metformin’s usage as first line drug, judging from its effectiveness, generally well-tolerated cost effective and long term usage. Health Benefits of Dark Chocolate Natalija Miller Get Your Daily Dose of Chocolate: The Darker, The Better! It has been reported that there are certain types of bacteria located within the human stomach that will actually ferment chocolate into heart-healthy, anti-inflammatory compounds. At the 247th National Meeting Exposition of the American Chemical Society, hosted at the Dallas Convention Center this last March, this conclusion was drawn and presented. Maria Moore who is surprisingly enough, an undergraduate student working on this research stated that two different types of microbes live within a person’s stomach. There are good ones like â€Å"Bifidobacterium and lactic acid bacteria†, which help to break down the chocolate, and then there’s the not so great ones such as some forms of Clostridia and E. coli, which have been known to not only produce inflammation, but also to potentially cause â€Å"gas, bloating, diarrhea and constipation†. She notes that, â€Å"When you eat dark chocolate, they [the good microbes] grow and ferment it, producing compou nds that are anti-inflammatory†. These researchers from Louisiana State University, led by John Finley, Ph.D., are the first to be conducting a study about dark chocolate’s effects on the multiple bacteria that reside within the stomach. It was said that when the compounds are absorbed, they effectively reduce the possibility of a stroke in later years by lessening cardiovascular tissue inflammation. By testing a total of 3 different types of cocoa powder in a mock digestive tract composed of â€Å"modified test tubes† used to mimic normal digestion, these researchers, â€Å"subjected the non-digestible materials to anaerobic fermentation using human fecal bacteria†. The main ingredient in chocolate, the thing that makes chocolate, chocolate cocoa powder, contains an abundance of polyphenolic compounds (antioxidants) catechin and epicatechin being two prominent examples and dietary fiber in a relatively small amount, both of which are â€Å"poorly dig ested and absorbed†. Finley then explained that, â€Å"In our study we found that the fiber is fermented and the large polyphenolic polymers are metabolized to smaller molecules, which are more easily absorbed. These smaller polymers exhibit anti-inflammatory activity†. Also, polyphenols found in the stomach may be converted or transformed to reduce inflammation by joining together prebiotics and cocoa powder fiber, increasing one’s overall health. Prebiotics can be defined as complex carbohydrates that cannot be broken down and digested, but the good bacteria in the stomach like to â€Å"eat† them. Finley then went on to say that, â€Å"When you ingest prebiotics, the beneficial gut microbial population increases and outcompetes any undesirable microbes in the gut, like those that cause stomach problems†. This article ended with a note on how dark chocolate could prove to be even more beneficial if combined with things like pomegranate, acai berry or other solid fruits (American Chemical Society, 2014). As far as how this concept pertains to the things that we have talked about in class this past year, we may need to dive a little deeper into the process than this article goes. Because we have yet to really learn anything about physio-chemistry which is where a vast majority of these concepts apply we’re going to have to talk in rather general terms. Considering the fact that the class is termed, â€Å"General Chemistry†, it would seem reasonable to do just that. First things first, we must consider a lab that was done rather early on in the year. While discussing solubility in class, a lab was done in which a certain solution was suspended in a test tube, combined with another solution and then heated until a solid was produced. This process formed something called a ‘precipitate’. Precipitate is define in the dictionary as, â€Å"to cause (something solid) to become separated from a liquid especially by a chemical process† (referring to the proc edure) or merely as a â€Å"a substance precipitated from a solution† (the solid itself) (Merriam-Websters online dictionary, n.d.). The other concept worth taking note of is thermodynamics. We have discussed Gibbs Free Energy in class these past few weeks. This too plays a role. A. D. McNaught and A. Wilkinson, the compilers of Compendium of Chemical Terminology, 2nd ed. (the Gold Book), refer to a process called Ostwald Ripening a process in which small, solid particles in a liquid compound are taken out of solution, while the dissolved species residing on the outside of those larger particles undergo redeposition. This process is one of the key underlying contributors to digestion. Food enters the stomach which is suspended (allowing the effects of gravity) placed in a solution of hydrochloric acid, potassium chloride, and sodium chloride (gastric acid) (Stewart, 1981), heated by the body, and then, through Ostwald Ripening, the food comes out as a larger, â€Å"purer/cleaner† (American Chemical Society, 2014), precipitated substance . One of the reasons why this process goes over so smoothly is because, â€Å"smaller particles have a higher surface energy, hence higher total Gibbs energy, than larger particles, giving rise to an apparent higher solubility† (McNaught Wilkinson, 1997). These concepts are important to take note of, with respect to the article about how great dark-chocolate is for a person’s overall health, because in order to understand how the chocolate is broken down and absorbed, there needs to be a foundation for how the process works in general. Understanding that the substance will dissociate in the stomach and the essentials for that operation, will open up a better foundation for someone to apprehend what the article was stating. The researchers concluded that chocolate contains two poorly digested materials. After the brief discussion on what digestion is, it can determined that chocolate has certain substances that cannot be precipitated. What then does the body do with what it cannot digest, or break down? Well, the answer according to the article is that they are â€Å"eaten†, fermented, metabolized and then absorbed by bacteria. In the case of cocoa powder, the products of this process have anti-inflammatory properties and can reduce the risk of stroke and stomach problems (American Chemical Society, 2014). Even though all the information on digestion was not included in this article, I thought what they did include was sufficiently put together, but in my personal opinion, to anyone who isn’t already knowledgeable about the subject in at least some capacity, would walk away from the article thinking nothing more than, â€Å"Sweet, dark-chocolate is good for me. I should eat more of it.† While this article is not necessarily promoting such action, it’s not opposing it either. The information, however, seems to be geared towards those who do, in fact, have an already established foundation for this type of material. I thought that the article was very reader-friendly and tried to give anyone curious enough to read about it a basic overview of the study, but I was ultimately left questioning more than I was before reading it. Because of what I have learned about thermodynamics, solubility and precipitation, I have to wonder a couple of things. For instance, how do good bacteria â€Å"eat† away the complex carbohydrates and dietary fiber and then ferment it? Is the process drastically different than digestion? What sort of heat is needed for that reaction to occur? I am interested in how, at the end of each process both digestion and fermentation/metabolization nutrients from the substance eaten are absorbed. How is the process of events different for each reaction? I have to wonder how these compounds have anti-inflammatory properties after absorption. What do those bacterium do to the dark-chocolate’s components, and ultimately, why isn’t the bacteria digested or fermented? Does it have to do with what reactions occur within the stomach? That would my best guess. These are all questions that I ask myself now because general chemistry has taught me a lot about reactions, what will react, what won’t and in what capacity. Because of that, I am left wondering more about what it is exactly that is happening in my stomach, not only when I eat dark-chocolate, but any time I put food in my mouth. Illusions and Realities in Ibsen’s Plays The Wild Duck and Ghosts In Ibsen’s The Wild Duck, illusions and reality are set into a conflict within the story of a son’s personal desire to confront idealism. Throughout much of the play, the son, Greger, argues the value of truth with the reluctant Dr. Relling. Relling insists on the importance of illusions, but fails to discourage Greger’s intentions and a play that begins as a comedy quickly turns into a tragedy because of these conflicts. At the heart of the illusions in this play are the ways that people assume many roles in a family, impersonating multiple ideals as ways for managing their relationships. This theme of impersonation is also developed in Ibsen’s Ghosts, where family relations are slowly undone as the illusions and deceptions are stripped away. In both plays, deceptions are strategic and designed to protect the children from the pains and struggles of their families’ histories. Ultimately, in these plays, families are held together by illus ions, yet torn apart by truths that have been concealed to protect the children. In The Wild Duck, as Relling continues to discourage Greger from revealing damaging truths about family secrets, Relling insists, "If you take away make-believe from the average man, you take away happiness as well" (Ibsen, 294). Relling is referring to the ways the Ekdal family is structured on particular deceptions; however, these are designed to protect the innocent as well as the guilty. Hedvig, the fourteen year old daughter, represents one of the innocents, and Greger’s father, Old Werle, represents a part of the guilty side. The key to these dualisms of false and truth, innocent and guilty, illusion and reality, lies in... ... necessary illusion. Both The Wild Duck, and Ghosts are tragedies that involve what might be understood as â€Å"the sins of the fathers;† however, Ibsen seems to suggest that some truths are better maintained as illusions. The key issue is to determine when Oxyglobin should be introduced to the market without jeopardizing Hemopure’s potential and how it should be marketed. In addressing the issue, the following were considered: a sensitivity analysis for potential consumption of different price series, associated revenues and costs, and gross profit from different distribution methods. It is recommended that Biopure: 1. Introduce Oxyglobin immediately at a price of $100 to veterinarians. 2. Have an independent sales force distribute the product to maximize profits. 3. Advertise Oxyglobin in both veterinarian journals and trade shows. 4. Establish a successful brand to launch Hemopure in the future. 5. Oxyglobin should be advertised Based on other Massachusetts companies’ initiatives that have not gotten FDA approval in the last few years, it is safe to assume that it is a possibility not to get the approval or to be given the approval rather late as it happened to Baxter. Further, the potential dip in the stock price if Hemopure were rejected can be avoided if the company acts quickly and takes advantage of a market that currently has no competition: the animal blood substitute market. Obtaining market share is critical now that Biopure’s Oxyglobin has been approved. Biopure has a chance to be the first player to enter this market and recover its research and development costs within two years. Even when Biopure was primarily focused on developing a human blood substitute, the opportunistic development and later approval of Oxyglobin, an animal blood substitute, is a valuable opportunity that has to be considered. Exhibits A and B place potential demand (1995) at 3.9 million units for noncritical cases and 0.35 million units for critical ones. Further, Exhibit C shows the probability of consumption in units after factoring in the probability that veterinarians and pet owners would try Oxyglobin at different price series. The table shows that at $100 a unit, Oxyglobin would be used in 81% of critical cases and in 28% of noncritical cases. Hemopure, the human blood substitute, was only about to enter the third phase of the FDA clinical trials. Only because Oxyglobin and Hemopure are almost identical in physical properties and appearance, it does not mean that they cannot be priced differently. There are currently different medical products and services for humans and animals that are differently priced. Exhibit 8 from the case shows that very few veterinarian procedures are priced over $100. In contrast, a human blood transfusion is priced above $1500 without insurance according to the Houston Memorial Hospital. Hemopure’s market consists of people who lose blood in large quantities like in accidents, gunshots. And and aging population (double of what it is today by 2030) in need of Red Blood Cells to treat certain conditions like chronic anemia and acute blood loss. Launching Oxyglobin at a low price would not necessarily create an unrealistic price expectation for Hemopure because human health care is far more expensive than animal care. Additionally, the emerging and growing necessity for blood substitutes will yield great demand in the following years which will translate to higher selling prices for Biopure, all things equal. Despite the fact that Baxter International launched the first human blood substitute, HemAssist (presumably priced between $600 and $800), Baxter International’s and Northfield Laboratories’ blood substitutes rely on outdated human blood as a source of hemoglobin which is more expensive than Biopure’s raw materials (bovine blood) at $1.50 a unit. Moreover, their products need to be frozen until used, while Biopure’s products are shelf-stable at room temperature. This makes Biopure’s products more advantageous because buyers can save money on refrigeration costs as the company makes a higher profit because of their raw material’s lower price. In terms of production, Biopure has capacity to produce only one product at a time, namely, an annual capacity of 300,000 units of Oxyglobin or 150,000 units of Hemopure or some linear combination of the two. On the other hand, Baxter has a production capacity of 1,000,000 Northfield’s capacity is at 10,000 units per year, which is considerably less than Biopure. According to Exhibit D, potential demand for Oxyglobin alone priced at $100 is 1.3 million. Biopure does not have any competition but cannot supply that level of output presently, which would potentially raise prices as demand skyrockets once the product is introduced in the market. Biopure conducted two surveys that showed customers’ willingness to try the product at a higher price depends on the gravity of the emergency. Exhibit E shows that by selling the product at $100 a unit, $137 million can be generated in revenue, which is more than the revenue that can be obtained when selling it at any other price. This has to do with the probability of consumption and its correlation to the gravity of pets situations in general. Veterinarians have expressed their frustration with current animal blood distribution. Biopure has two options distribution options: National-Regional-Local and Independent Sales Force. Comparing distribution costs from Exhibits F and G, it is clear that going for the independent sales force approach Biopure can more than reduce costs by half. Distributing the product (at $100 a unit) with the NRL option would cost around $41 million as opposed to $18 million with independent sales distribution. Exhibits H and I show that the highest gross profit attainable is $120 million when using an independent sales force and pricing Oxyglobin at $100 a unit for veterinarians. These and all exhibits show that Biopure should launch Oxyglobin immediately and recoup the $200 million developing costs in only two years.
